JOB TITLE: Jordan’s Principle Case Manager
TERM: ASAP – March 31, 2019 (possibility of renewal)
POST: IRTC Sub-office
SALARY: TBD (based on education and experience)

Job Description:.
Under the Supervision of the Jordan’s Principle Service Coordinator, the Jordan’s Principle Case  Manager will work closely with the Jordan’s Principle Wellness Coordinator and the Jordan’s  Principle Wellness Coordinator. This team’s main objective is to build capacity in the  communities while assisting Member Community Jordan’s Principle front-line workers in ensuring  children and their families have access to the health and social services they need, when they need  them, and to ensure that they do not experience denials, delays, or disruptions of services due to  jurisdictional disputes.

The Jordan’s Principle Case Manager’s main focus is to provide support to Community Case Mangers in
the following (but not limited) areas:

– Assist in coordinating comprehensive client services
– Participate in developing strategies, policies, procedures and program evaluation
– Assess the deficits with service systems and the need to protect children  through early intervention, sharing of information, effective co-ordination of services and  appropriate training
– Assist in developing work plans and updating service delivery plans

Qualifications:
– BN/RN, RN, LPN Social Worker, Physiotherapist, Occupational Therapist or Teacher
– CPR and First Aide Certification
– Proven understanding of First Nations culture, health care & referral process to children’s rehabilitation programs
– A very high degree of accuracy and attention to detail
– Excellent Case management and facilitation skills
– Effective verbal and listening communications skills;
– Strong leadership, critical thinking, decision making and problem-solving skills
– Proficient computer skills (Word, Excel and PowerPoint)
– Ability to develop partnerships with communities and organizations
– Ability to work independently and with minimum supervision
– Must be available to work flexible hours and be able to travel
– Criminal Record Check and Child Abuse Registry Check must be submitted with resume
– Must have valid driver’s license and own vehicle or access to a vehicle
– Preference will be given to qualified Manitoba Interlake First Nation members
